A feedback QR code opens your survey at the moment a customer has an opinion: on the receipt, the table card, the packing slip or the exit door. It links to a Google Form, Typeform or SurveyMonkey survey and is static, so it never expires. It is free with no sign-up, made in your browser, and verified by a real scanner before you download it.

How to make a feedback QR code
- Build the form and copy its link. Google Forms: Send → the link icon → Shorten URL → Copy. Typeform: Share → Copy link. SurveyMonkey: Collect responses → Web link.
- To know which branch or table the answer came from, pre-fill a field: in Google Forms choose Get pre-filled link, fill in the Location question, click Get link and copy that address. Typeform and SurveyMonkey use hidden fields or custom variables added to the link.
- Paste the address into Feedback form link and click Test link to check the form opens without a login.
- Wait for the badge to read Verified, then choose Download PNG for the receipt printer, Download SVG for the table card designer, or Download PDF with a caption.
Feedback QR code details
A feedback code is a link code, and the link is a survey. Scanning it opens the form in the phone’s browser; the person answers, submits, and the response lands in your spreadsheet or dashboard.
Getting the link right
Every form builder offers a share link. Google Forms can shorten it to a forms.gle address, which makes a smaller code and does not lapse while the form exists. Make sure the form does not require sign-in, or every scan ends at a login screen.
Pre-filled fields for location
One survey can serve several places if the link tells the form where it was scanned. In Google Forms, add a short-answer question called ‘Location’, open the menu, choose Get pre-filled link, type the branch or table name into that question, and copy the link it produces. It is a long docs.google.com address ending in something like entry.123456=Table+7, so the code is denser than a forms.gle one. Typeform does the same with hidden fields and SurveyMonkey with custom variables, both added to the link as ?location=table7. Where the code works
- Receipts: a 2.5 cm code with ‘How did we do?’ printed by the till.
- Table cards and counter tents, alongside the Wi-Fi or menu code.
Scan the printed card with the QR code reader to confirm the pre-filled value survived.
Common mistakes
- A form still in edit mode, or one that has stopped accepting responses.
- A pre-filled link copied from the edit view rather than from Get pre-filled link.
- A code on a shiny laminated card under a spotlight; use matte.
This code never expires
The form link is stored inside the code, so the code keeps working for as long as the form accepts responses. This is a static QR code: the data is written into the pattern, so scanning it needs no server, no account and no subscription. There is nothing to renew and no redirect that can be switched off, which is why a code made here keeps working for as long as the printed copy exists. The trade-off is honest: a static code cannot be edited after printing and does not count scans. If you need either, encode a link to a page you control. Styling, formats and print size
Every code can be styled in the Style panel: six dot shapes, corner shapes, solid or gradient colours, a transparent background, a quiet zone of 0–8 modules, and your own logo up to 30 percent of the width. A contrast checker warns below 3:1 and blocks downloads below 1.5:1, and after each change the preview is decoded again so the badge tells you whether the styled code still scans.

Print size: as a rule of thumb the code should be at least one tenth of the scanning distance wide, with the quiet zone included.

A receipt or a table card is scanned from 20–40 cm, so 2–4 cm is enough; a pre-filled link is longer than a plain one, so do not go below 2.5 cm for those.

Frequently asked questions
Does a feedback QR code expire?
No. The code stores the form's address itself and is generated in your browser. It stops working only if you close or delete the form. A form that has stopped accepting responses shows a message, so reopen it rather than reprint.
Which form builder should I use?
Any that gives a public link: Google Forms is free and exports to a spreadsheet, Typeform is pleasant on phones, SurveyMonkey has reporting. The code does not care. Choose by what you want to do with the answers, then keep the form short enough to finish quickly.
How do I know which branch or table the feedback came from?
Pre-fill a location field in the link. Google Forms has Get pre-filled link in its menu, Typeform has hidden fields, SurveyMonkey has custom variables. Make one link per location and one code per link. The answer arrives with the location attached, without the customer typing it.
Can I put a feedback code on receipts?
Yes. Receipt printers are low resolution, so use a short link, download the PNG at 512 px, and let the till software scale it to 2.5–3 cm. Test the printed receipt with a phone; thermal paper prints slightly grey, and the code needs a clear white margin.
Can I see how many people scanned the code?
Not from the code; a static code has no server to count scans. The form's response count tells you how many finished, and a pre-filled location field splits that by place. For scan counts, add UTM tags to a link on your own site that forwards to the form.
Is a Google review link better than a feedback form?
They do different jobs. A review link asks for a public rating on Google; a feedback form asks for private, detailed answers you can act on. Many businesses use both: a feedback code on the receipt, a review code at the counter, on separate cards with clear captions.
